I understand that for paper scripts the original copy must be brought in when collecting to mark off the dispensing. A scanned copy of a paper script is not a legal document and the original must be presented for pick up. For postage patients, the original paper script must be posted to ‘Reply Paid 88377, Aspley QLD 4034’ before your order can be dispatched.

This risk assessment form only needs to be filled out once. Repeat prescriptions dispensed by Aspley D&N Pharmacy will not require you to fill this out again for repeat dispensing’s. This information is entirely confidential and only used by pharmacists to ensure the quality use of medications.
